Transferee and Returnee

Transferee and Returnee should submit the following requirements to the Admission Office:

        • 2 Photocopy of PSA Birth Certificate

        • 3 pieces 2" x 2" recent colored ID Picture

        • Copy of the Report Card with LRN ( 1 Original and 2 Photocopies) duly signed by the school principal.

        • Recommendation Form (Grade 4 to 12 only)- (placed in a sealed envelope)

        • Certificate of Good Moral Character from the Guidance Counselor or School Principal (From Grades 4 to 12 only).

        • Baptismal Certificate/Certificate of Dedication (for non-Catholic)

        • Marriage Certificate (if parents are married) - OPTIONAL

        • ESC Certificate from previous school for ESC Grantee Only (Incoming Grades 8 to 12)

        • 2 pieces Long Folder

        • 1 Long Brown Envelope

*All documents should be placed in a long brown envelope labeled with name written in pencil.
