Preschool

          Preschool applicant should submit the following requirements to the Admission Office:

• • • • • ENTRANCE Exam (Kinder only)

        • 2 Photocopy of PSA Birth Certificate

        • 3 pieces 2" x 2" recent colored ID Picture 

        • Copy of the Report Card with LRN (1 Original and  2 Photocopies) if the applicant undergone schooling

        • Baptismal Certificate/Certificate of Dedication (for non-Catholic)

        • Marriage Certificate (if parents are married)- OPTIONAL

        • 2 pieces Long Folder

        • 1 Long Brown Envelope

*All documents should be placed in a long brown envelope labeled with name written in pencil.

          For Nursery, the applicant should be at least 4 years old on or before October 31 of the current year.

          For Kindergarten, the applicant should be at least 5 years old on or before October 31 of the current year.
